MEMBERSHIP AGREEMENT

HOLD HARMLESS AGREEMENT

Milwaukee GAMMA, Inc. (GAMMA) is a not-for-profit organization operated entirely by volunteers for the purpose of bringing together adult men and women interested in athletic, outdoor, recreational, social and cultural activities. Organizers and participants should keep in mind that some activities could be physically demanding and even dangerous. Neither GAMMA nor the organizers of the individual events assume liability for any injury which may result from participating in an activity. Participants must assume responsibility for their own conduct and safety.

Thus, applicants for membership, and current members of GAMMA, are asked to agree to the following:

Release, Assumption of Risk and Indemnification Agreement

In consideration for my acceptance as a member in GAMMA, and with the understanding that my membership in GAMMA is upon the condition that I enter into this agreement, for myself, my heirs and assigns, I hereby assume the risks inherent in participating in certain events and activities, including sporting competition and activities that may be inherently dangerous. I expressly assume the risk of and accept full responsibility for any and all injuries (including death) and accidents, which may occur as a result of participating in the events and activities of GAMMA, and I release from liability Milwaukee GAMMA, Inc., and each of it officers, directors and managers, agents, representatives, coordinators and volunteers. I waive any claim I may have as a result of any and all injuries to my person or property as a result of my participation in the activities and events of GAMMA. I agree to indemnify Milwaukee GAMMA, Inc. for any and all claims, including attorneys’ fees and costs, which maybe brought against it by anyone claiming to have been injured as a result of any injury to me or my property, which may occur as a result of Milwaukee GAMMA, Inc. activities and events. I further agree that the Membership Directory I will receive is for my personal use and is not to be used for commercial purposes or shared with non-members.

I certify that I have read and fully understand this release. By electronically submitting my membership application, I indicate this.

Privacy Statement

Milwaukee GAMMA, Inc. takes the privacy of its members very seriously. We never distribute members' personal information to any outside or commercial organization. We have a strict policy against distribution of a member's email address, phone number, or any other information. Your email address and other personal information will NEVER be sold or given to any organization.

Milwaukee GAMMA, Inc. does publish the following: a monthly newsletter called GAMMA Rays, an Annual Membership Directory (w/mid-year update) and a web-site.

Newsletter: The monthly newsletter is distributed to members and contains information relating to events, including the host members' names, address, etc. There are also photos of events and activities. If you would prefer to not have your photo appear in our newsletter, please follow the Opt-Out procedure below. A special edition of the newsletter is occasionally distributed at events such as PrideFest. For these occasions, members' personal information is not included.

Website: Our website is available for public viewing. No personal information is published on pages available to the public, although photos of GAMMA events are posted. If you'd prefer to not to have your photo posted on the website, please follow the Opt-Out procedure below. We also maintain pages for members only which are password protected. These pages are available so that members can check event information online.

Directory: Our Annual Membership Directory is distributed to members only. If you'd prefer to not have your information printed in our directory, please follow the Opt-Out procedure below.

Opt-Out: If you'd prefer that your information NOT be included, please let us know. The general areas that people opt-out of are:

Not appearing in photos in website

Limited information in directory. In this case, tell us what you wish to have deleted by putting the information in parenthesis i.e.:
Richard S(mith)
or by telling us to delete your address, phone, email, etc.

To Opt-Out just send us a message indicating your wishes, or call the Membership Director at (414) 530-1886. If we do not hear from you, we will assume you are not opting out.
